In 2011 we went to Colo Rectal Surgery department,Severance Robotic Institute, Yonsei University,Seoul and improvised our techniques of Robotic surgery of Rectal Cancer from Right hemicolectomy to Ultra Low Anterior resection to Exenteration to Lateral Pelvic node dissection. Thanks to Prof .KIm and Prof.Byong Son Min whose techniques were very methodical.
Other than Surgery ,I learnt humbleness from the Koreans. Being updated in technology and teaching others are very important in our short life on this Earth.
And we didn’t keep what we learnt with us we went as Mentor and trined many Surgeons who were interested. From AIIMS to SGPGI to Adyar CMC ,Apollo branches ,Fortis and regularly give cadaver training at MS.Ramaiah Medical College Lab.
Apollo Cancer Center,Apollo hospitals Hyderabad has the most advanced and the latest DaVinci Xi Robot with facilities to do all these Surgeries and we can use Firefly, Sureform staplers etc.,

Our team has started the robotic surgeries 15 years ago. The colo rectal cancers are increasing in India and more in younger patients. The role of minimally invasive surgical approach is increasing and we found equivalent oncological clearance in terms of TME and lateral nodal clearance with lesser morbidity. Especially post radiation cases, high BMI cases, very low rectal cancers we have found the robotic approach better than laparoscopy.
And while mentoring these cases across India in many centres, we received feed back in various challenges while doing low anterior resection. Main challenge was splenic mobilisation and pelvic dissection during the same docking. The straight line ports is difficult to follow and can cause more difficulty in splenic mobilisation. Hence we MODIFIED PORTS AS IN THE PICTURE
And we are teaching the same approach in the hospitals or Institutes we are called to mentor. And the feedback we are getting from them is – ITS SIMPLE, REPRODUCIBLE.We have standardised this technique and will try to cross check again, make modifications as and when needed.
The posts are placed in oblique fashion as given in the picture and we will be able to do all the steps without hopping or dual docking.
